Welcome Uniteen Leaders and Directors:
When creating your Unitreat registration forms packet for the Uniteen and the Adult participants, please follow these directions completely.

Print one set of of all the documents and note that they are numbered in order to simplify the process for you.

When printing, paper clip the files together so that when you are making your copies, the double sided forms and forms 4 pages long print as a group correctly.

Note the "Parents Information Handout" has blanks for you to fill in to inform the parents of the date you select for fee and form return and your contact information. Be sure all parents and chaperon's receive this document as it tells them what to expect, what to bring and what NOT to bring.

Note the "Registration Instructions" handout which was created to assist you in understanding what documents are required for each participant and
what documents are required for the group as a whole.

Incomplete forms and missing documents will cause delay in your groups registration process. 

Payment MUST accompany the registration forms.  Ministers must sign to confirm participation eligibility.  No exceptions.

In addition, YOU SUPPORT STAFF has separate forms available under the YOU SUPPORT STAFF tab on the Uniteen page of the SCUCA website.  First, an application to serve on the YOU SUPPORT STAFF TEAM is required which has an earlier due date.  If selected as a member of the Team, the YOUer will be required to submit their registration, medical & heart agreement to you with their fee and travel to the event with the Uniteen group.  There is a space to list YOU Staff on the "Group Registration Form".
